Appointment Cancellations & No-Shows

Patients are required to give at least 24 hours’ notice for cancellations or rescheduling. If a patient fails to attend a scheduled appointment, they are subject to a fee.

Late Arrivals

If a patient arrives late, the treatment time will be shortened to avoid delays for subsequent appointments. If a patient arrives more than 15 minutes late, the appointment may be considered a no-show, and they may be asked to reschedule and incur a cancellation fee.

Payment

Payment is due at the time of the appointment. Acceptable payment methods include debit, credit, cash, and e-transfer (osteolistica@gmail.com). If payment is not presented at the time of the appointment, an additional fee of $25 will be charged to the patient.

Treatment Consent & Informed Consent

All patients will be required to sign a consent form before treatment, acknowledging that they understand the nature of the treatment and any potential risks. The practitioner will explain the treatment plan and answer any questions before beginning the session. This ensures the patient is fully informed about the treatment they will receive and consents to them voluntarily.

Intake Forms

The health history intake form must be completed at least 12 hours prior to the appointment time booked with your OMP. If the intake form is not completed prior to your appointment, you will be asked to complete it in the office which will take time away from your treatment.

Referrals

If a referral from another healthcare professional is required, it should be obtained before beginning treatment.

Respectful Behavior & Professionalism

All patients are expected to maintain respectful behavior toward the practitioner and other clients. Any inappropriate behavior, including verbal abuse, threats, or harassment, will result in the termination of the treatment relationship.

Post-Treatment Care & Advice

After treatment, patients will receive guidance on how to care for themselves. Patients are encouraged to follow the advice provided and should consult their OMP if they experience any issues or discomfort after treatment.
